Welcome to AZ Sleep. This website, azsleepclinic.com (the “Site”), is operated by Janne M Lynch DDS PLLC, doing business as AZ Sleep (“AZ Sleep,” “we,” “our,” or “us”). These Terms of Service (“Terms”) govern your access to and use of the Site.
By accessing or using the Site, you agree to be bound by these Terms. If you do not agree with these Terms, please do not use the Site.
Our Services
AZ Sleep evaluates and treats obstructive sleep apnea and snoring at our offices in Glendale, Scottsdale, Sun City and Sedona, Arizona. Our care includes:
- Our Free Sleep Assessment screening questionnaire
- Visits with our nurse practitioner
- Home sleep testing, ordered and reviewed by our nurse practitioner
- Custom oral appliance therapy with our dental sleep team
- Follow-up testing and care
Not Medical Advice
The articles, videos and other information on the Site are for general education only. They are not medical advice and do not replace an evaluation by a qualified health care provider. The Free Sleep Assessment is a screening tool, not a diagnosis. Using the Site or submitting a form does not create a provider and patient relationship. If you think you are having a medical emergency, call 911.
Eligibility
By using the Site, you confirm that you are at least 18 years old, or that you are using the Site with the involvement of a parent or guardian.
Privacy and Data Security
We take privacy and data protection seriously.

- Patient information is treated as confidential and protected by appropriate administrative, physical, and technical safeguards.
- How we use and protect your health information is described in our Notice of Privacy Practices.
We do not sell your health information. We do not share it with third parties except as described in our Notice of Privacy Practices, as required by law, or with your written permission.
Please do not send detailed medical information through the Site’s forms. We will contact you to collect what we need in a secure way.

All content on the Site, including logos, designs, text, graphics, images, videos and software, is the property of AZ Sleep or its licensors and is protected by copyright, trademark, and other applicable laws.
